BHI Training Series: Leveraging BH & Support Staff for Pts w/Chronic Health Conditions

Description: This 2-hour session will discuss how to leverage behavioral health and clinical support staff to develop a more integrated care approach to treating patients with chronic conditions. The session will review the relationship between health behaviors, behavioral health, and chronic conditions; provide an overview of the basis of habits and behavior, with effective strategies to replace old habits with healthier new ones; and describe evidence-based practices supporting behavior change that can be used by the entire care team to support effective behavioral management of patients with chronic conditions.   

 

Presenter: Amber Murray, RN, BSN, MA, PMP is a Senior Technical Expert Lead and Project Director with JBS International. She serves as Acting Director for the HRSA-funded Behavioral Health Technical Assistance grant, which provides free primary and behavioral health integration training and TA to HRSA-supported health centers. She’s worked with community health centers and PCAs across the country to adopt an effective integrated care culture, analyze and adapt workflows to enhance care efficiency, and support implementation of integrated care best practices. Previously, she worked as a Registered Nurse in both inpatient and outpatient settings across several medical disciplines and is a Certified Health and Wellness Coach, supporting patients with chronic disease.
