Join us for our upcoming webinar, Communication & Team Integration: How CHWs Can Effectively Present Cases to Providers, on Tuesday, March 10, 2026, from 12:00 PM to 1:00 PM. We’re excited to be joined by Ligia Nuñez, OBAT Program Coordinator at Kennedy Community Health Center (Kennedy CHC), who will lead this interactive session.
This session is focused on helping CHWs feel more confident and effective when presenting patient cases to providers, especially in the context of diabetes and hypertension care. During the session, Ligia will talk through practical ways to bridge clinical and non-clinical perspectives, communicate clearly with care teams, and still keep the patient’s voice and lived experience at the center.
Topics will include:
- Practical communication strategies for presenting patient cases to providers
- Motivational Interviewing (MI) tools (including OARS) for navigating challenging conversations
- Provider vs. CHW approaches to gathering information and what makes a strong patient report
- Documentation tips, case examples, and real-world workflow practices
- Team dynamics, CHW-provider relationship building, and brief mandated reporter guidance
While the forum is CHW-focused, all members of the care team are welcome and encouraged to attend, especially those who work closely with CHWs or support care coordination.
