Join us for our upcoming webinar, Communication Strategies: How CHWs Can Effectively Present Cases to Care Team Members, on Monday, April 13th, 2026 from 12:00 PM to 1:00 PM. We’re excited to be joined by Ligia Nuñez, OBAT Program Coordinator at Kennedy Community Health Center (Kennedy CHC), who will lead this interactive session.
This session is focused on helping CHWs feel more confident and effective when presenting patient cases to providers, especially in the context of diabetes and hypertension care. During the session, Ligia will talk through practical ways to bridge clinical and non-clinical perspectives, effective strategies for CHWs to communicate clearly with care teams, while ensuring that the patient’s voice and lived experience remain central.
Topics will include:
- Practical communication strategies for presenting patient cases to members of the care team to providers
- Motivational Interviewing (MI) tools (including OARS) to strengthen communication
- Provider vs. CHW approaches to gathering information and what makes a strong patient report
- Documentation tips, case examples, and workflow practices
While the forum is CHW-focused, all members of the care team are welcome and encouraged to attend, especially those who work closely with CHWs or support care coordination.
