Dr. Hafsatou Fifi Diop (Assistant Commissioner of Health Equity, Massachusetts Department of Public Health) will present on maternal health data and health promotion priorities of the Department.  Dr. Diop is a founding member of the Perinatal-Neonatal Quality Improvement Network (PNQIN), the statewide perinatal quality collaborative for Massachusetts. Dr. Diop's presentation, "The State of Maternal Health in Massachusetts: The Intersection of Public Health Data, Clinical Practice and Policy," has the following learning objectives:  (1) By the end of this webinar, participants will be able to identify inequities in key maternal and child health data; and (2) participants will learn how to use data to change clinical practice and make policy recommendations to improve maternal and child health. During the webinar, we will also have an OB Module demonstration by Azara DRVS.

This presentation is the first of three webinars in this series, scheduled to take place in April, August, and December 2025.

Hafsatou Fifi Diop, MD, MPH, is the inaugural Assistant Commissioner of Health Equity at the Massachusetts Department of Public health (MA DPH). In this role, Dr. Diop is tasked with overseeing, coordinating, and harmonizing the work of the Office of Health Equity and Community Engagement, the Office of Problem Gambling Services, the implementation of the department’s Strategic Plan to Advance Racial Equity, and the Advancing Health Equity in Massachusetts (AHEM) initiative. Prior to this position, Dr. Diop served as the Director of the Division of Maternal and Child Health Research and Analysis at DPH. Dr. Diop is a founding member of the Perinatal-Neonatal Quality Improvement Network (PNQIN), the statewide perinatal quality collaborative for Massachusetts. She served as co-PI of the Massachusetts Maternal Mortality and Morbidity Review Committee (MMMRC) for 19 years. As PI and co-PI on multiple grants funded by CDC, HRSA, and NIH, Dr. Diop has a strong track record of contributing to science through the many publications she has co-authored. Dr. Diop received her medical degree from the University of Conakry, Guinea in 1990. She completed the 21st International Course of Epidemiology held at the Institute National De La Santé Et De La Recherche Médicale (INSERM) in Paris and did her field practicum at the Head Quarters of the World Health Organization (WHO) in Geneva, Switzerland in 1996. She received her Master of Public Health degree with concentration in MCH in 2000 from the University of Hawaii at Manoa.

Leah Dafoulas, MPH, is the Director of Clinical Transformation at Azara Healthcare where she works with practices to improve quality through the use of data and technology.  Leah is passionate about the profound role of data-driven analytics to improve population health and transform patient care. Prior to Azara, Leah was an Account Manager at a start-up health technology company focused on transitions of care where she worked with hospitals and health plans to optimize the use of a data analytics solution. She began her career at Pfizer where she ran population health management projects with ACOs and hospitals. Leah has an MPH degree from Boston University and BA and MA degrees in Biology from Clark University. She also has a Six Sigma Green Belt certification. 

Samuel Bar, Director of Solutions Engineering, joined Azara in 2014 bringing with him valuable experience from his service in the Peace Corps and a strong background from his prior career in manufacturing process improvement. He was attracted to Azara's data-centric approach and its mission-driven work for its healthcare safety-net clients. In his previous roles as Implementation Specialist, Implementation Business Team Lead, and Support Manager, Sam utilized his extensive knowledge of Azara’s products and processes to implement tools and advocate for product enhancements that address the needs of both current and future users. As the Director of Solutions Engineering, Sam is responsible for understanding the challenges faced by customers, recognizing their significance to patients, providers, and health plans, and identifying how Azara can effectively address these issues. He holds a Bachelor of Science in Industrial Engineering from Northeastern University.
