Join us for an upcoming webinar, Chronic Disease Management: How CHWs Can Support Use Motivational Interviewing to Coordinate Care on Thursday, June 18th from 12:00 PM to 1:00PM. 

 

Lead by Harvard Thompson from the Center for Health Impact, this session will focus on applying Motivational Interviewing (MI) skills in both clinical team interactions and patient care planning. Participants will learn practical strategies to support diabetes and hypertension self-management and coordinated chronic disease care.

 

Session objectives will include: 

  • Understanding the CHW role in chronic disease management 
  • Identifying barriers to diabetes and hypertension self - management 
  • Support care coordination utilizing MI skills in a culturally responsive way

While the forum is CHW-focused, all members of the care team are welcome and encouraged to attend, especially those who work closely with CHWs or support care coordination.

 

CHWs will receive 1 CE upon attending the full session.
