This webinar will provide an overview of the updated reporting requirements for NCQA PCMH 2025 renewals and standard revisions, including timelines, revised criteria, and process for submission. After the webinar, participants will:

  • Have reviewed the 2025 Annual Reporting Requirements
  • Have reviewed the Initial and Annual Distinction of Behavioral Health Integration Reporting Requirements process 
  • Learned about the assessment tool for evaluating the health centers’ compliance in achieving the PCMH Core Competencies
