Supporting Diabetes Self-Management for Older Adults: Lessons from the PACE Model

Join us on Tuesday, May 19 from 12:00–1:00 PM for a virtual learning session on how the PACE model supports diabetes self-management for older adults.

Emily Schilling from Upham’s PACE will share an overview of the PACE model, how interdisciplinary teams support older adults living with diabetes, and practical examples from the PACE setting. The session will also include discussion of how diabetes technology, including CGM and other tools, can fit into workflows for older adults.

This session is intended for community health center staff, diabetes champions, care team members, and others supporting patients with diabetes.

CEUs will be offered, pending approval, and the session will be recorded.
