Small Moments Big Impact is a website that helps you accomplish this during the visits following birth. It is being disseminated nationally but we wanted a special focus at CHCs because it helps bridge the connection when a provider is from a different cultural , economic or racial background. Brief videos of mothers telling their stories serve as a model and stimulus for your patients/parents to talk about their experiences and aspirations ; this is the new well child with a healthy baby.
